Every time you attend a clinic to donate platelets you will be given a copy of our Health and Lifestyle Questionnaire to complete. It is vitally important that you answer each question with honesty and accuracy to ensure that it’s safe for you to donate and that it’s safe for a sick person to receive your platelets donation. All information that you provide is confidential.
